PermissionsEx help

I have tried to create a pex file repeatedly and have also copied several from differenet sources to no avail. All pex files i add to my server keep showing up red on my plugins list in game. If someone could possibly make a basic pex file that they know for sure will work and post a link in the comment, or message me it, I would greatly appreciate it.
