I went onto a server recently, as per usual, in search of playing and having fun, etc. Several hours (2) of going about, minding my own business, a moderator appears nearby. He said he wouldn't hurt me, so I let him stick around 'cause, hey, it's a mod. So I traversed the land in search of a place where a friend and I could begin building without many disturbances, the moderator still behind me. When I finally decided on a spot, the Moderator began building a bedrock cocoon around me. I managed to escape through the top or into the water about twenty times before I just thought, "Screw it" and let him trap me before warping back to spawn. At this, he said he'd ban me for no particular reason. Instead, he just jailed me and spawned probably 100 dogs in with me so my game crashed whenever I tried to log in again. I finally decided to just delete the server and leave a comment stating who was corrupting the game, and now I'm here to give insight on what I believe should be appropriate circumstances for electing moderators, admins, etc.
(1) ~ Do NOT give away higher ranks for money. Despite your need to keep the server up, at least get to know each person and learn how they act and treat others over the course of a few days/a week.
(2) ~ Especially don't give away power/rights to people who ASK for it. I know this is stated in many blogs and on many rule boards in-game, but it can't be emphasized enough. If they nag you too much, kick or ban. That simple. Otherwise they may go out on a destructive rampage.
(3) ~ Again, get to know the people playing on your server before you upgrade their status and power. Check your server log to see how people react to each other while you're away. How they speak to one another can hint at whether they're worthy of higher status.
I know this isn't much to work with, but anybody who knows what I'm talking about should understand completely. And without further adieu, I bid thee goodnight. . .

If an owner is too young or immature their server generally always fail because of how they act and who they put at top spots

i generally only promote people who have been palying teh server for a long time or donated a generous ammoutn of money(cuz hey who wants to abandon money)
The moderator's there seem like complete A**holes. However those three points you mentioned about how to prevent this are almost exactly the rules I follow when I choose my own moderators for my server. I do admit we do abuse some times when we get bored but we don't abuse to the point where we make players leave, We usually try get other people involved if they wish to freely and have some fun with us
