Is there a way to make a natural generated world with these characteristics or would I have to manually make it?
One medium sized island ( about 5000x5000 )
With a bunch of smaller island darted around the rest of the map( wich is all ocean)( they range from 1000x1000 to 100x100)

Alas, you'll probably need to download a third party program, because I don't believe that is possible to do in regular MC or even in non-advanced minecraft generation plugins/mods (though not too sure about the mod part... but you're in the server section, so :/)
Even advanced ones probably require a third party editor.
Only other option I can get off the top of my head is what ya said - manually making it. Though worldedit does make the job easier.
It would be best to look for the answer on google before taking this for granted, though.

I'm thinking of probably using world painter to make the world and the basic shapes for the islands and then use world edit for the finer details and etc
