Server to Server Platform Teleporting

Hello I am creating 2 servers and was also going to have a HUB server so players can teleport from server to server. Does anyone know of an UPDATED plugin that teleports players to another server by platforms or portals without using any client-side mods?

An example of what I want is the server IP: mc.levelupmc.com

I would like one just like that. If you can help and you are a mature, good player, you can become staff on my server.
