Craft a „Worm Heart“ (Sorry i dont have ideas for a crafting recipe) and dropp it into an fossil structure (either a new worm fossil structure or the vanilla structure

  1. Find a 'worm fossil' structure, which will generate randomly underground or overground in deserts (if possible)
  2. Throw a Totem of Undying at the fossil's head area
  3. Behold biggo worm
You could also make a nether variant in soul sand valleys
