I'll try be as descriptive as I can
To make this you don't need a crafting table
Place soul sand as the first block, put another block of soul sand on top of that, and 2 blocks of soul sand either side of the top one. The put Wither skeletons skulls onto the 3 highest soul sand blocks. Once you have placed the last skull, it will turn into a wither, so be ready - set up a confinement space, and have all your good armour on.
