Worldguard Bypass

So recently a hacker with a strange name like this faction_cd00292e_29e6_4214_9a09_9a44a6c91e71 joined my server and was able to build and break on my worldguarded land. I have double checked and i am certain that breaking blocks and building is turned off. The hacker also didnt have op and was just a regular player. Does anyone know why he was able to bypass worldguard?

Also double check the player's permissions. If a staff member has the ability to alter perms they could have added it to a friend. Make sure you and only you can alter perms. Also its possible that there was a backdoor set up like a unsafe plugin, mod-pack, etc that the user had access to to get the permissions required to do that.
Does the server have the offline mode setting on true? if so then players without proper minecraft accounts can join, change the server settings to offline mode:false and people like that will not be able to join.
