Alter the /give command?

Hi, I am running a server with some custom items, and I was curious ad to whether it would be possible to alter the /give command so I can use it to give custom items, for example, instead of typing /give @p music_disc_11{CustomModelData:1}, I could type /give @p music_disc_custom. Would this be possible to do?
