My game crashes every 5-40 minutes i don't know why please help me.

Considering there are mod packs with 200 + mods included, 70 doesn't seem so bad.
As a start, remove optifime, which will leave all your game content intact. If the game plays ok without optifine, you could look at other performance enhancing mods like Foamfix :

If trying without optifine makes no difference, from the look of your crash, I'd look at the lycanitesmobs mod as a likely suspect. Perhaps try removing it and create a new map, leaving your existing one intact so that you can go back to it if it turns out to be some other mod. If you can play your new map for a longer period, we might be on to something.

I tried without optifine, it crashes immediately so that's bad. I have no problem with preformance. When we fix this would you like to play this modded minecraft with me?
We have to get it going first .........

I think i have fixed it, i removed tf2 mod and it works fine no crashes so far.
First of all, I have no idea how you are even running that many mods but I think its the lost cities mod.
Well, i took out the most important mods from my custom modpack originaly i had more than 100 mods but same situtaion as in that modpack it crashes every 5-40 minutes. It's not lost cities i tought it was extra planets mod it's addon for galacticraft but it's not. So help me.
