Server Voting Problem

Ok so if you try and vote on my server at my voting link: (Ad link removed)
It doesnt't work because it doesn't ask for the username. I don't know why, all the voting things are setup correctly, is there anything i can do to fix this?

Well, your link was removed so maybe you should just explain the platform you're using and how you set it up and maybe someone could help you then. But for now, no one knows enough to even try.
