Whats the name of your YouTube?
And yeah you will need some 3D modelling software. Blender is free and great but you can always use Cinema4D if you have the money.
Mate I really can't make an intro for you because I don't have #ProSkillz in these things. However you may really not want to make an intro from scratch. Just download a template of the web and replace the default skin with your skin and change the text to your name. Maybe watch some tutorials and you will get a clear idea if no one is out there to help. Hope I helped.
