Optifine doesn't require forge, if you run the optifine mod file, it will add an optifine profile to your Minecraft Launcher.
Optifine also gives you cool things like dynamic lighting, which allows light to be produced when holding an object that gives off light (i.e torches or glowstone dust) and you can also run shaders and special resource packs that change entity models and items.
