Off-model skins incorporate personal headcanons, that may involve changing the shape, species, or bodily features of a Minecraft character.
On-model skins don't do this, but do change the outfit or coloration of a character. Even drastic outfit/color changes, if not meeting off-model criteria, fit in this category.
