For me, it was the dumb villagers going to open the door of an NPC village and then a creeper came in and exploded. lol.

Yes it is inside, but you can't craft it. You need to use a MCPE Inventory Editory to get it. And it's buggy since it takes picture of the smoke coming out from the camera. (So it's a picture of smoke, and some background.)
