If you are on the same computer, launch your minecraft client and connect to 127.0.0.1 (which means, the same computer).

Making a backup and completely re-installing Minecraft generally solves this problem. Do not forget to properly delete all the minecraft related files under your "App Data" folders.

Honestly, the Wiki is very well documented for PermissionsEX. Maybe if you had some more specific questions, people could assist you better.

Did you try to close Filezilla and re-initiate a new connection? You have to understand that FTP Sessions will timeout after a certain amount of time and automatically (dis-connect you). You must then re-login/re-connect.

It is possible that by accident two instances are running.... Did you look at your processes to make sure that a lingering old process is not running and causing the conflict?
