So you can put a command block under the ground and in it put the command:
tp [the player you want to teleport] [coordinates of the place you want to tp them to]
After that place a dirt block (or any other block you want) above it.
Then put a pressure plate above the other block and you should make it blend with the environment.
When the player steps on the pressure plate, he will tp to the place you want.
