No I have not been banned on any server.
I would be a good moderator because I believe that people who do wrong should be give chances, but if they don't adhere to it they should be punished. I also believe that a happy community is best for making a server grow based on my past experience and moderating.
I would help the server by making it a environment where people would enjoy playing with one another.

I can be on for 3-5 hours (Weekdays) 7-9 (Weekends)

I totally agree with you guys. Some people are really too much for me. I was on a prison server (IP withheld) and they had an anti advertisement policy so this guy started passing out books with the server IP in it and he just threw one at me without me knowing. When the staff did a check, they found the book and started questioning me and when I told them they mocked me and banned me.
