Worst thing thats happened to me in the nether was when i was with my friend on a server (we had diamond armor and diamond swords...) and 3 ghasts attacked us. My friend ran away and i was trying to shoot the ghast and hit a pigman instead... there were heaps of pigmen in the area and they all attacked me and picked up my armor and sword! when i came back to fetch my items, i noticed that the pigman with my stuff was swimming in a lake of lava and was unreachable...
