what would you do if someone greifed?
I would ask around, and if he/she did grief, I would ban them for one month.(By then, the griefer will have forgotten about the server) If they came back and griefed again, I would Perm-ban them.
What would you do if someone didn't follow rules?
I don't mean to be Literal, but that would depend on which rule would be broken. If it was hacking, and I witnessed it happen, I would perm-ban them. If it was something like being overly-rude(or verbally abusing another player), I would warn them, if it happened again I would ban them for a day, then a week if again.
