I forget the name somewhat, but the first videogame I ever played was a game on my dad's old computer when I was 4. It had something to do with there was a penguin named Tux, and you tried not to let him fall while he was sledding down a snow mountain.

I'm not sure if it counts as a videogame though, since it was more of a minigame. But it's as early as I can remember that might count.

An island is corrupted by a deadly virus, you must enter the most corrupted area and destroy it with TNT. Also there is a corrupted city inside, you must rebuild it after destroying the virus. The most corrupted spot would be deep in the island under a shell of netherrack.

So that there was more decorating options, like chairs that you can actually sit on. Also tables and vases, and possibly a chess set for multiplayer and singleplayer, or the option to get XP from more than just fighting mobs. Or a flying mob, and more farming options.

I don't believe Herobrine ever existed in the first place...it could have been another player, and the person taking a screenshot or videotaping might have been lying. Also, those pictures of supposed "sightings" could be edited to have Herobrine in them.
One other thing: how could a mob who makes TNT traps and pushes you off cliffs be in the game without the players knowing? Herobrine could only been modded or edited. He's a complete fake.

I would really like a bird mob. It would be speckled, blue for ocean, green for jungle and orange color for desert. At dawn and dusk, it would fly to a tree or cactus chosen when the bird spawned. It would lay a speckled bird egg, and players could take the egg and place it somewhere else to lure the bird. It could be plucked when right-clicked, to get a rainbow feather. It would drop turkey, and the eggs could be smelted into rainbow syrup, used to lure cats when placed on a tree. The syrup could also be crafted to make a trampoline which would blast you 10 blocks high.
The bird doesn't have a name, but I'd really like to see this or a trampoline in minecraft.

When I can't seem to find any coal or ores. I have to wait the night on a few dirt blocks with no light, while monsters swarm around the far-from-finished dirt hut.

My biggest fall was when I was playing Skyblock with a friend, and I was trying to build a bridge to the desert sky island. There was a zombie at the end of the bridge. Both of us were frantically throwing snowballs at it, since we had no swords. (we weren't expecting to run into any mobs yet). The zombie saw me, and in a frantic dash to the grass sky island, it punches me off and I fall 5 times.
If you add up the length of the sky 5 times, that's quite a fall.
Sadly, I wasn't taping at the time.
Just noticed this thread is on biggest "fails", not falls.

Here's a possible reason why people don't play legitimately:
1. They really don't want to put a lot of work in it, thereby ruining the fun
2. They are impatient and want to have things done right then
3. They just want to feel cool, by supposedly "cheating the game."

Impatience is normal though, that's why I usually don't make huge projects.

I would cover it and everything else in a mound of End Stone, then challenge my friends to find and dig it up. Basically, dragon egg scavenger hunt.

When you know someone is lying but they keep denying it. Especially when their reasons keep getting more unreasonable. It's really frustrating, why can't they just tell the truth?
