MeTotem is a program that generates a resource pack that replaces the Totem of Undying texture with a small version of your own skin. The way that this works is that you enter your username into the program, and it uses a skin API to get a front portrait of your skin as well as your head for the pack icon. It then replaces the totem texture with this front portrait and packages a Java and Bedrock pack to suit your preference.
