On the top shelf: diamond blocks on the left and right and a nether star in the middleon the middle shelf: a nether star in the middleand on the bottom shelf a stick in the middle

Go to %appdata% (type in your start menu) and go to .minecraft. then go to your bin. then right click and open with winrar(or 7-zip) and then delete META-INF (very important to delete it) and then drag the stuff into that.
