Honestly just hold Left Shift when selecting a world type (Superflat, amplified, ect.) until you get to Debug Mode. Its much easier then downloading this map...

Also, If you have any mod installed the world will include every block existent within that mod(s).

Be advised, it is an only spectator mode world.
