I present to you Discworld, from Terry Pratchett's Discworld novels.
The Discworld is a disc-shaped planet that sits on the backs of four elephants, which in turn stand on the back of the giant sky turtle, the Great A'tuin, which swims through space. The disc contains four continents. These are as follows. The (Unnamed) Continent includes the Ramtops (the mountains at the center), Lancre, Ã berwald, and of course the great city of Ankh-Morpork. The Counterweight Continent is the smaller continent (towards the turtle's head in this project) and is made mostly of gold to balance with the larger continents on the other side of the disc.
Klatch, "not loosely based on Africa at all," is largely desert, but does have some mountains and jungle. The last is XXXX (say "fourex"), a mysterious continent of which little is known.
