I don't remember for sure but it is ether - JolliCraft or GoodMorningCraft.
In order to achieve the grass effect on the side of the blocks and clear water - u need to patch your minecraft client with a MCPatcher also.

I can't share the whole world file, because lot's of other projects are hosted on our server. But I will copy paste it to a new world and share it in a few days.

It is Jollicraft texture pack.
Additionally minecraft is modded with "MCPatcher" (for grass, lava and water improvements).
