Been Messing around for awhile with a teleporter system for my Ultima Online project to get a player to all the different moongates and after searcing and not finding anything simple, I came up with this.
You have to right click the command block and input the x,y,z in this format "tp @p[135,19,-172] 161 65 203"
the135,19,-172is the pressure plate location. the161 65 203is the location you want to be teleported to
*Updated to an even more compact design, dont even need any of the redstone wire or torches.
