Oomukade Genome (オオムカデゲノム) is a form of Kamen Rider Revi, based on Kamen Rider Kabuto and a centipede.

Oomukade Genome is accessed using the Oomukade Vistamp (オオムカデバイスタンプ) in the Revice Driver (リバイドライバー). Revi gains the ability to charge up his punches and kicks with electricity, increasing their power. By focusing the electricity into an internal generator, Revi can go into a "Clock Up" state, moving at high speeds for a short period of time.

When Remix is activated, he combines with Vice into Revice Oomukade (リバイスオオムカデ), a large centipede. Revi grabs onto Vice's legs while lying down, centipede legs flip out on his arms chest and legs while Vice forms the head and pincers.

Designer notes:
The Kabuto Horn is modified to be a giant centipede with the jaws wide open. The centipede design continues down onto his chest, with the tail being in the center. On his arms and legs are armor pieces that can flip out for the Remix form, and under his arms are also pieces that flip out to form centipede legs.
