Really? But I've seen everywhere else of her as a girl, and even her sprite in-game clearly shows a girl.

Have you heard his in-game voice? It is clearly male when he says "Ooh yes..." in his transformation into EX. And his in-game sprite is a Rock and Roll performer, not a female. He is also referred to as male in the game, which practically confirms he is male.

Well the wiki uses male pronouns, so I think Mettaton is a guy. 

Now his over world sprite does indeed look like a girl.
