If you open the model in BB, it has no texture. It has a file on the left that it's looking for, but it's missing as it's not where it should be. If you change the location of that file to the texture/item folder, it will work
To make it simple, open up the model in BB. Right click on the texture listed on the left. Click replace file. Click open the items folder and select the sword texture. Export model and save over old one. Done
You simply have the model looking in the wrong place. It seems like you had the texture in the textures folder, made the model, then threw the texture in the texture/items folder. The problem there is that the model doesn't know to look in the texture/items folder so it's still looking where it used to be
