Before I start, I want to say that I have been on hundreds of Minecraft servers, observing staff, ranks, and general server issues, and also have owned my own server.
Another observation that I have come across, is the new Purchase Unban feature many servers feature on their Buycraft or DonationCraft page. This is usually only on bigger servers.
I am truly disappointed in these servers, because it not only proves their staff are incompetent and lazy to a stretch, it also tells the players that they don't really care about the h4xors, spammers, etc. that make the server unenjoyable being banned or not. It shows that they just want donations and more money for themselves (come on, admit it, I'm not going to lie and say I don't want to earn a little profit from my server, which I do) but there comes a point that it's plain obvoius they're a bit greedy.
How do we fix this? There's really no solid solution to this, as it is their server and they can choose which way they want to control it. But do you really want to join a server with staff that made the server to maximize donations? Do you want to join a server with spammers, hackers, griefers, or whatever just buying unban and breaking the rules all over again?
Chances are, is that you probably don't, so I believe that we should all strive to play on servers with truly dedicated staff and devoted to making our play-experience the best possible.
