Dragon Heart
While in the top right slot of your inventory it will activate every 20 seconds giving you Regen 3 for 5 seconds

Wither Ash
While in the top right slot of your inventory it will activate every 15 seconds it will give all mobs exluding players
a wither 3 effect for 3 seconds.
The time for this effect is divided by each player standing together around the same mob.

Warden Guts
While in the top right slot of your inventory it will activate every 20 seconds giving mobs in a 20 block radius glowing for 5 seconds

Elder Eye
When placed in an item frame on top of a beacon the beacon will now give a mining fatigue effect
based on the level of the beacon

If I ever find out how to make a resource pack that changes things based on their name, I'll probably add it. For now I'm okay with it using vanilla item textures

you can do it with Optifine's CIT or
you can use vanilla CustomModelData, but you need to specify that in the command
