It makes things disappear, and makes it really hard to do anything. I've tried Optifine and that didn't help. What should I do?

Try moving towards it so it will depict the image. That same thing happened to me. It is usually caused by lag.
But it keeps coming back. It started about a week ago. I hadn't played it since then, and when I got back on, it was still doing this.

No. That's vanilla. I usually use Soartex though.

It is the correct 64 bit version. I have reinstalled Minecraft, and force updated. I personally don't have mods, but the server that I play on has some. I don't know what all of them are, but I do know one of them is iClaims.
Server mods don't affect your client, they only affect the server.

As far as I can tell, yes, it's only in multiplayer. But I've asked other people on the server and no one else has this problem.
It's probably just a problem in your network connection, since it's only in multiplayer. Or it could be that the server your on doesn't have enough RAM?
I'm almost positive it's not a connection problem. I'll have to check with the admins about the RAM.
