Skulls can have attribute modifiers just like any item, getting the skull is different than just diamond boots however. If you just want a normal skull its /give @p skull 1 3 {SkullOwner:PlayerName}

/give @p skull 1 3 {SkullOwner:majora,AttributeModifiers:[{Operation:0,UUIDLeast:1,UUIDMost:1,Amount:10,AttributeName:generic.maxHealth,Name:hbonus},{Operation:1,UUIDLeast:1,UUIDMost:1,Amount:0.5,AttributeName:generic.movementSpeed,Name:sbonus}]}
That'll do a bonus 5 hearts of health, which is what I'm assuming you meant, as well as the speed.
