Axel Form (アクセルフォーム) is an upgrade for Kamen Rider Faiz.

Kamen Rider Faiz accesses this form using the Faiz Axel Watch. In this form, the chestplate flips open, revealing the Faiz Core (ファイズコア) on his chest. Kamen Rider Faiz can move at the speed of sound, but can only remain in this form for a total of 10 seconds. His physical attacks are all enhanced, and he can perform multiple Exceed Charges consecutively.

Kamen Rider Faiz Axel Form has the following finishing attacks:

• Axel Crimson Smash (アクセルクリムゾンスマッシュ): Using the Faiz Pointer, Kamen Rider Faiz jumps and creates multiple drill-like projections on the enemy. Faiz then kicks into one of the projections, causing all of the drills to drill into the enemy and destroying them.
• Axel Grand Impact (アクセルグランドインパクト): Using the Faiz Shot, Kamen Rider Faiz punches the enemy with enhanced speed.
• Axel Sparkle Cut (アクセルスパークルカット): Using the Faiz Edge, Kamen Rider Faiz slashes the enemy with enhanced speed.
