I've made much more sophisticated models but this is my first public pack. Of course.

2. PMC will show a caution warning for a link redirect. Click on the button labelled as "CONTINUE"
3. The site will show a (safe, non malicious!) ad for 5 seconds. After those 5 seconds, you can hit "SKIP"
4. You should arrive at a MediaFire page to download the pack!

To get the items, you need to have the pack downloaded and loaded into your game of course. You need to then run the command
/give @p minecraft:carved_pumpkin[custom_model_data:#] where the # is the number in the description associated with the color of cat ears you want to get! Unfortunately, there's not a way to get them in survival without extra download stuff, you need commands for it. But I hope that you have fun with the ears anyway!
