Because his mother stripped him from everything and because he is in a basement battling monsters and versions of himself

  To quote the intro:
  
  "'I will do my best to save him, my Lord,' Isaac’s mother replied, rushing into Isaac’s room, removing all that was evil from his life."
  
  This is then followed by Isaac's mom taking away his gaming handheld (likely the gamekid), a doll (presumably Mr. Dolly, but without any hair), and Isaac's clothes.
